Filling Out a Customer Profile Form

To download a Customer Profile form, just right-click the link below and select the command "Save Link As..." or "Save Target As..." to save it to your hard drive. When you have saved it, double-click the document and it will open as a Microsoft Word template file.

You will see a number of short grey boxes. Click on the each one and type in the requested information. Please note - some of the boxes are pull-down boxes - these are the ones that already have information in them. To use these boxes, click and scroll down to the appropriate option. Then move on to the next box.

When you have finished, save the document with your name, attach it to an e-mail message, and send it to info@brookshiretravel.com. We'll enter your information in our database which will help us better serve you. We look forward to hearing from you!
